Outpatient Mental Health Clinic

Through our Outpatient Mental Health Clinic (OMHC) program, we work with youth, adults and families to provide treatment for mental and behavioral illness. Our goal is to work with each individual to provide treatment, education, and tools needed for the individual to function at their optimum potential. We achieve our goal by providing the following services within the OMHC program:

Any one referred to our OMHC program will complete an intake session. This also serves as an opportunity for the individual to ask our trained intake staff about what to expect from the OMHC program and to learn about the other programs and services that are available at OHS Inc.

Usually, at the same visit when the intake is completed, a licensed Professional Counselor, or Social Worker sits with the new client to complete a Psychosocial Assessment to evaluate the individual’s mental, physical and emotional health. This helps them determine the individual’s strengths and weaknesses, as well as their community resource and treatment needs.

The Psychiatric Evaluation is used to provide a mental and behavioral health diagnosis for the client and is typically completed by a board certified Psychiatrist or Psychiatric Mental Health Nurse Practitioner (PMHNP).

The evaluation also includes mental and behavioral history which is based on the client’s description of their symptoms and behaviors, and how these symptoms and behaviors affect their daily functioning in various areas such as school, work,  family, and relationships.

The Individual Treatment Plan (ITP) is used to define measurable goals that the individual hopes to achieve while they are enrolled in the OMHC program. The ITP is created in working sessions with the new client, or the client’s parent/guardian, and their therapist. The  ITP includes:

  1. The client’s strengths
  2. Long term goals
  3. Measurable short term goals that moves the client towards achieving their long term goals
  4. Target date for each goal

Individual and family therapy services are provided by licensed Professional Counselors and Social Workers. During these sessions, the therapist works with the individual on the goals that have been identified in the client’s Individual Treatment Plan (ITP). The frequency of therapy services is determined by the client’s assigned licensed therapist and is based on their treatment needs.

We offer medication education, prescription, monitoring, provided by a board certified Psychiatrist or Psychiatric Mental Health Nurse Practitioner (PMHNP).

Our group therapy sessions consists of several participants and is moderated by a licensed Professional Counselor or Social Worker. In group therapy, participants are able develop social skills and learn how to articulate their issues while receiving encouragement and support from other individuals who may have had similar experiences. Any stories or information shared by a participant are kept confidential by other participants, and participants are reminded of this confidentiality in every session.

Some of the groups we have offered include:

  • How ya’ Doin’?: For women to discuss life’s challenges and offer each other support in overcoming those challenges.
  • Anxiety Support Group: For adults to learn how to manage symptoms of their anxiety.
  • Grief Support Group: For individuals who have lost loved ones to share their struggles and challenges, and to receive companionship and understanding from others.
  • Coping Through the Quarantine: For clients to receive emotional support virtually during the stay-at-home order due to the Coronavirus (COVID-19) pandemic.

Referral & Collaboration

Our OMHC services are not provided in isolation: we frequently evaluate each individual to determine whether they would benefit from other programs and services.

We provide of these services within our Psychiatric Rehabilitation Program and Case Management program. In the case that our clinicians determine that an individual would benefit from services that are not provided by OHS Inc., we promptly connect them with those external services, and continually collaborate with the service provider to make sure that our clients are getting what they need.

Additionally, we collaborate with schools, the Department of Social Services, Child Protective Services, as well as other health and social service organizations to ensure our clients are getting the best treatment and services available.
